Purpose

To recognize the professional nursing achievement of an alumnus of the Cornell University-New York Hospital School of Nursing.

Objectives

1. To identify graduates of the School of Nursing who have distinguished themselves in nursing.

2. To acquaint the public and the alumni with the achievements of the award recipient.

Criteria for Nomination

1. The individual should have a record of achievement in one or more of the following fields in nursing:
* Nursing Practice
* Nursing Service
* Nursing Education
* Nursing Research
* Teaching

2. Significant professional contributions should have been made in one or more of the following ways:

* Publishing
* Lecturing and program participation beyond that of the usual job requirements
* Original research (this may be based on doctoral dissertation)
* Clinical studies which have led to the improvement of patient care

Supporting Data

1. Minimum of two letters of support-one from a non-alumnus, one from an alumnus.

2. Curriculum Vitae-updated within the past 12 months.

3. Sample of publications when available—no more than three need to be submitted.
